Against that backdrop, practices that don’t actively track their cost to collect are essentially flying blind on one of the most consequential indicators of operational health.

    Cost to collect pulls together several distinct expense categories. Labor costs typically dominate, covering salaries and benefits for billing staff, coders, denial specialists, and patient financial services representatives. Technology costs follow closely, including EHR systems, practice management platforms, clearinghouse fees, and analytics tools used to monitor performance. Third-party costs, outsourced RCM, collection agencies, consulting fees, can also weigh heavily, sometimes more than an equivalent in-house team would cost. Administrative overhead, office space, management time, and compliance training, rounds out the full picture.

    The formula is simple even though gathering the inputs can be complicated: total revenue cycle expenses divided by net patient revenue, multiplied by 100, gives you cost to collect as a percentage. Industry benchmarks generally place a healthy range between 3% and 6%, with top performers clustering near the lower end thanks to strong collections, low denials, high clean claim rates, and consistently compliant processes.

    Why Benchmarks Need Context

    A raw percentage means little without proper context. Smaller practices often run higher percentages simply because fixed overhead spreads across fewer claims. Larger systems frequently achieve lower per-claim costs through volume and more robust infrastructure. Specialty mix matters too, since coding complexity and payer rules vary considerably across specialties, as does payer mix, since government payers and complex managed care contracts tend to increase administrative burden. The only meaningful comparison is against organizations of similar size, specialty, and payer composition.

    What Pushes the Number in the Wrong Direction

    Several recurring issues consistently drive cost to collect higher. Rising denial rates create rework that delays payment and increases workload. Manual data entry introduces errors during eligibility checks, coding, and claim submission. Complex insurance plans require disproportionate staff time on calls and appeals. Inadequately trained staff generate more errors, while overstaffing inflates payroll without improving results. Aging receivables require repeated follow-up touchpoints. And unclear patient billing communication shifts collection effort later into the cycle, when it’s harder and more expensive to recover.

    Concrete Strategies for Improvement

    A handful of proven strategies consistently reduce cost to collect over time. Confirming eligibility and authorizations before the visit improves first-pass clean claim rates immediately. Outsourcing persistent bottlenecks, like denial follow-up or aging accounts, can resolve balances faster than an overstretched internal team managing everything simultaneously. Automating routine tasks like claim scrubbing and payment posting reduces manual error rates. Tracking KPI trends regularly, denial rate, collection rate, days in receivables, helps leadership spot operational problems while they’re still manageable. Standardizing workflows across the revenue cycle improves consistency and simplifies onboarding new staff. And communicating cost estimates clearly to patients upfront reduces the volume of follow-up billing work later.

    Cost to collect isn’t a vanity metric; it’s a direct window into how efficiently your entire revenue cycle operates. Calculating it accurately, comparing it against genuinely comparable organizations, and systematically addressing the factors driving it higher gives practices a concrete, measurable path toward stronger cash flow and more sustainable financial performance, even as the broader reimbursement environment keeps getting tougher.
